Now there's more space junk in orbit to worry about.

Any speculations on what caused the rupture/explosion and now more orbiting space junk than ever before? At this rate, if just one of these fragments hits another satellite the cascading effects beyond this may become exponential and it won't be pretty.
Any thoughts? Comments? Ideas?
